A Lisbon, Portugal court just ruled that rather than the “official” 17,000 COVID deaths reported by the health ministries, only 152 actually died of COVID, and the rest died “with” COVID, meaning they blew a positive on the deeply flawed PCR test that is useless for diagnosis, per its inventor, and which will be mostly false positives over 27 amplification cycles (the standard around the world has been more like 35-45, where inactive or virus fragments are picked up due to a doubling of resolution with each cycle, which do not signify an infection, but rather an encounter with the virus that was defeated by one’s immune system, or never even gained purchase and turned into anything).

There may be a more benign explanation, which is that Covid-19 as such is not a cause of death. Cardiac arrest or respiratory arrest is, for instance.
On a death certificate, this immediate cause should then be followed by underlying and contributing cause(s), such as a Covid-19 infection. So, if the question is what was recorded, specifically, in the 'immediate cause of death' field of a death certificate, then we shouldn't expect to find Covid-19 there, except in error.
I suppose that these "Instructions for Completing the Cause-of-Death Section of the Death Certificate", by the CDC, are representative of how deaths are recorded internationally:
